SUBMISSION OF FILLED UP FORMS: 

Ø      Candidates are requested to fill up the forms legibly and neatly using blue or black ballpoint pen(dot pen).

Ø      Forms filled up and duly signed by the candidate and father/guardian will have to be self- attested after proper scrutiny as mentioned in the form.

Ø      Filled up forms are to be submitted to the college office only on such dates and time as will notified by the college authorities.

Ø      Requests for acceptance of filled up forms after the expiry of prescribed date for submission of forms will not be entertained.

Ø     A photocopy of Higher Secondary or equivalent Mark Sheet has to be attached with the form.

Ø     Following documents are to be produced in original for verification at the time of admission:

1)     Admit Card/Madhyamik Certificate as proof of age

2)     Mark Sheet of HS or equivalent Examination

3)     Caste Certificate for candidates applying for the reserved categories.

4)     Physically Handicapped Certificate by any government hospital clearly showing percentage of handicap.
